Before your child can attend our program, he/she must have a State of IL Dept. of Human Services Medical Form and a Lead/Tuberculosis Form signed by your pediatrician.

Print the DHS Medical Form (2 pages) and the Lead and TB Form (below). All three pages need to be completed and signed by your child’s health care provider. Please CHECK to make sure that your heath care provider fills out both pages of the DHS Medical Form and signs the first and second pages. (The only parts they can skip are “Alternative Proof of Immunity” and “Vision and Hearing Screening data” at the bottom of page one.)

Parents need to fill out their student’s name, birth date, etc. at the top of both page one and page two of the DHS Medical Form. In addition, the top third of the second page (all of the yes/no questions) need to be completed and signed by you.
